Tell it the site, the work, how many operatives and how long, and you get back a hazard register with likelihood and severity scored one to five, controls, and a residual score that is lower than the initial one because the sums were actually done. Then you edit it, because it is your document and your name goes on it.

Who read which version, and when

Assign it to the people who need it, they read it and sign, and the document carries its own record of that. Amend it and the version moves — so the question is never "did they sign the RAMS", it is "which version did they sign", and that has an answer.
